NORTH COUNTRY, N.Y. -- A North Country company has been cited by OSHA after a deadly construction accident.

Occupational Safety and Health Administration investigators looked into the death of 23-year-old Kyle Brown. He died last November.

Brown was blown off a Pulaski car dealership's roof while trying to install metal decking.

OSHA says Watertown contractor MTL Design Inc., had employees using harnesses that were not properly installed.

Investigators also say the company failed to train employees on fall hazards and failed to secure the decking against the wind.

OSHA issued four citations and is proposing a fine of nearly $89,000. ClientIP:, UserAgent: CCBot/2.0 ( Profile: TWCSAMLSP
